Scholarship Application – Hoffman Institute

Applications are considered on a first-come, first-served basis, and funds are limited for each Process. Please apply as soon as possible and indicate which Process dates will work best for your schedule.

Application Instructions:

Complete this entire application thoroughly and thoughtfully (unclear or incomplete submissions will delay scholarship determination). If you need to leave the form for any reason before submitting, click the orange "Save and Continue Application Later" button at the bottom of the window and then either copy and save the retrieval link, or enter your email address to send the link to your email. This will save your progress so you don't have to start over.
Provide a complete copy of your most recent income tax return. This is required in order to review your application.
At the bottom of this form, attach your income tax return (including Schedule C or equivalent, if self-employed) and "Submit" your application. This online form is on a secure website to ensure your forms are transmitted safely.
